Medicare, Medicaid, and State Assistance Programs

As the cost of health care continues to rise, more and more Americans are feeling the pinch.

According to the Kaiser Family Foundation:¹

43% of adults with health insurance have difficulty affording their health insurance plan deductible.
One out of three Americans have trouble paying their insurance plan premium or other cost-sharing measures, such as copays or coinsurance.
29% of adults have problems paying their medical bills. 73% of those people have cut back on spending money for food and other basic items as a result.
27% of Americans have delayed getting health care, 23% have skipped seeking care altogether and 21% have skipped filling a prescription because of the cost.

Fortunately, there are a number of resources available to people who might have difficulties affording health care.

Here is more information on some of the national resources that can help with health insurance and health care costs. We’ve also included a state-by-state guide of more localized resources in your community.

What is Medicaid?

Medicaid is a program that is jointly funded by individual states and the federal government. Medicaid provides health insurance coverage for people who have lower incomes.

As of 2023, nearly than 90 million Americans had Medicaid, making it the single largest health insurance program in the U.S.²

Each state has its own rules regarding Medicaid eligibility and coverage.

Medicare Medicaid Dual Eligibility (D-SNP)

“Dual eligibility” means being qualified for both Medicare and Medicaid.

If you are Medicare dual eligible, you may qualify for a Dual Eligible Special Needs Plan (D-SNP).

Dual Eligible Specials Needs Plans are a type of Medicare Advantage plan that pay for part or all of your Medicare deductibles, copayments and coinsurance. These plans can also provide benefits for things not covered by Medicare, such as routine dental, vision and hearing care.

Medicare Savings Programs

A Dual Special Needs Plan is not the only way to receive assistance with Medicare costs.

A Medicare Savings Program (MSP) is a federally funded health insurance program that is administered by the state where you live.

A Medicare Savings Program can help pay for Medicare premiums, deductibles, copayments and coinsurance.

There are four types of Medicare Savings Programs:

Qualified Beneficiary Medicare (QMB) Program
This program helps pay for the Medicare Part A and Part B premium along with deductibles, copayments and coinsurance.

If you get QMB benefits, health care providers aren’t allowed to charge you for any services or items that Medicare covers. Pharmacists can only charge you a small amount (no more than $4.30 in 2023) for prescription drugs that are covered by Medicare Part D.

Individuals can qualify for a QMB with a monthly income of less than $1,235 in 2023, and married couples may qualify with a combined income of less than $1,663 per month in 2023.
Specified Low Income Medicare Beneficiary (SLMB) Program
The SLMB program helps pay for the Part B premium and is available to individuals who have an income of less than $1,478 per month in 2023, or couples with an income of less than $1,992 per month in 2023.
Qualified Individual (QI) Program
The QI program helps pay for the Part B premium. You can’t get QI benefits if you also qualify for Medicaid.

The 2023 qualifying income limits are $1,660 per month for an individual and $2,239 per month for a married couple.
Qualifying Disabled and Working Individuals (QDWI)
The QDWI helps pay for Medicare Part A premiums.

QDWI benefits are available to working disabled persons under the age of 65 with a monthly income of no more than $4,945 in 2023 or resources valued at less than $4,000 in 2023. Benefits are available to married couples with income limits of $6,659 per month in 2023 or a resource limit of $6,000.

The income limit amounts listed above may increase in 2024. If your income or resources are close to these amounts, you should still apply.

You automatically qualify for Medicare Extra Help if you qualify for the QMB program, the SLMB program or the QI program.

Medicare Extra Help

Extra Help is a federal program that helps pay for some of the out-of-pocket costs associated with Medicare Part D (prescription drug coverage).

These costs include:

Part D premiums
Part D deductibles
Part D copayments

Extra Help is also known as the Medicare Part D Low-Income Subsidy (LIS).

If you get Extra Help in 2023, your prescription drugs can’t cost more than $4.50 for each generic drug and $11.20 for each brand name drug that is covered by your Part D plan.

You may qualify for Extra Help in 2023 if you have less than $21,870 in annual income and less than $29,580 in resources. A married couple may qualify in 2023 if they have an annual income of $16,600 or less and less than $33,240 in resources.

“Resources” include things like stocks, bonds and money in a checking or savings account. It doesn’t include things like your home, a car or other household and personal items.

Children’s Health Insurance Program (CHIP)

CHIP is the Children’s Health Insurance Program, which provides low-cost health insurance to children in families who earn too much money to qualify for Medicaid. In some states, CHIP also provides coverage to pregnant women.

Each state has its own requirements for CHIP eligibility and coverage.

State Health Insurance Program (SHIP)

SHIP is the State Health Insurance Program.

This state program receives federal funding to provide counseling, education, training and outreach for Medicare-eligible individuals and families.

The goal of SHIP is to help people make more informed decisions regarding their health care.

Housing Assistance

High medical bills can make it extremely hard for people to pay their rent or mortgage.

There are numerous resources available to help make housing more affordable for people who have lower incomes. If you can get help paying for some of your housing costs, you may be able to better afford the health care you need.

The Department of Housing and Urban Development (HUD) helps to improve and develop communities and enforce fair housing laws.

Public Housing

Previously known as Section 8 housing, housing choice vouchers provide a way for people to better afford housing in the private market. Vouchers are paid for through federal funds and eligibility is determined by income.

The state of Alabama offers a wide variety of resources to its residents over 65 who are looking to purchase a Medicare plan.

Alaska

Alaska does not offer Medicare Advantage plans (Part C), but there are still other options for you to explore your Medicare coverage options and have your questions answered.

Arkansas

California

California Department of Insurance (CDI) – This is the general CDI website that provides a wealth of information about Medicare and health insurance in general.
California Department of Aging – This department provides programs to help seniors and those with disabilities. For assistance in your county, contact your local Health Insurance Counseling and Advocacy Program (HICAP) office.
California Health Advocates – Medi-Cal and other support programs provide help with paying for certain Medicare costs.
250% California Working Disabled (CWD) program – To qualify, you must be working, disabled, and not qualify for Medi-Cal.

Colorado

Colorado provides several resources to help residents navigate Medicare.

Colorado Department of Regulatory Agencies — Find SHIP counselors, Medicare basics, and prescription drug benefit information.
Low Income Assistance Program — The Extra Help/Low Income Subsidy can help pay your Medicare Part D premiums, deductibles and copays.
Old Age Pension Health and Medical Care Program (OAP) — If you make too much money to qualify for Health First Colorado and receive Old Age Pension, you may be eligible for the OAP Health and Medicare Care Program.

Choices Program – Also known as the CT SHIP, this organization provides help and counseling to seniors about health insurance, including Medicare.

Delaware

Delaware offers several Medicare Assistance programs and a State Health Insurance Program that offers counseling and other services to Medicare recipients. The following sites provide more information about Medicare and other resources.

Delaware Medicare Assistance Bureau (DMAB) — Formerly known as ELDERinfo, DMAB provides free health insurance counseling for Medicare recipients (including those who are under 65 years of age).

District Of Columbia

Washington D.C. offers a few resources to Medicare recipients in the district, and there are a few options that seniors can explore for more information about their options.

DC Medicaid – Qualifying residents of the District of Columbia can receive help paying for medical services from doctors, hospitals and pharmacies who are enrolled in the program.

Maryland
